I've looked at various doctors forms online to see the kind of layout most commonly used. I want to design my own one of these for the scene. The layouts shown below are very grey and straight forward, to create authenticity within the scene I am designing for I will need to follow these traits to an extent - it will need to be legible and easy to fill out, although as I am designing for made up world and time, it can be a lot more fun.
